Context = Assad's Loyalist faction is winning. Rebels are infighting. Al Quaeda are trying to become/already are (depending on who you believe) the big Rebel player in Idlib.
It could be, in no particular order:
Rebels doing it because they want to make it look like Assad did it and garner support.
Rebels doing it because they're fighting each other.
Loyalists doing it because he thinks he can get away with it.
Loyalists hitting a rebel chemical weapons factory with a normal bomb and releasing the toxins stored there.
Turkey false flag based conspiracy
ISIS for whatever mad reason
All are definitely well within the realms of reason, which tells you a lot about the conflict.
